: No, the Judicial Council doesn’t have a model form for the Assignment of Judgment document, just a suggestion of what it should contain.

: So you’re left to making one up yourself. You might go to your local law library and ask the reference librarian if she can locate a model in one of their forms books.

Check out the "judgments" article in California forms of pleading and practice, a multivolume set you will find in the law library. There should be a form there you can follow.
