Filling in the Affidavit of Deed

Zachary,

Yes, you're right the name of the grantee on the Grant Deed you'd fill in on #1 and #2D on the Affidavit of Deed form.

The party named as the grantee on the grant deed would be the same that you'd list on #1 and #2D of the Affidavit.
