Jim,

The forms you'll prepare will depend on whether you've got enough time to utilize the services of both escrow and title.

If you're going to do the deal without the involvement of escrow and title, then in addition to the forms you mentioned, you'll need a Notice of Cancellation, an Affidavit of Deed, a Promissory Note and Trust Deed and probably an Addendum or two.
