Previously reconveyed TD now under default

Good day to everyone. While looking at a 2nd TD on a property at the recorders office for a possible JBB, I came across a surprising reconveyance recorded (2 yrs prior) for the TD that I thought was the current 1st TD. Bit confused because this reconveyed thought-to-be 1st TD now has a recorded NOD on it (as of last week) which indicates it is actually alive and well.

Perhaps my eyes were playing tricks on me but I did double check the recorded ID# against each other. It is almost Halloween after all. A data service I take also shows the preliminary title still has the 1st TD that I saw the reconveyance on. Hope that makes sense...in any case does it sometimes happen that a reconveyance on a TD is recorded by mistake? I saw no other recordings that indicated the reconveyance was a mistake.
